I know the code to write my office add-in, but I don't know how to start. The web site says something about a Napa Valley tool  which I can't find. Do I use Visual  Studio or what tool do I use to create the actual add-in?

If you use Visual Studio the easiest way to get started should be this guide. You don't need to worry about Nappa or anything else outside of Visual Studio. If you don't have the option to create a new office add-in project in Visual Studio you should make sure you have the office development tools selected in the Visual Studio installer as they may not be installed per default.
