I was wondering if someone could tell me how to set up a screen. I have all of the account information entered into the database, but am not certain on how I am to insert it into a switchboard/form. Is their a module that someone has handy?

It''s not very clear what you''re after, but if I''m close to understanding you, the Form Wizard will help you produce what you''re after. If I''m off the mark then you may have to clarify the question somewhat.
